Paycheck Protection Program (PPP) Information

 

The following are the qualification conditions for new customers:

  1. Business Located in the State of Arizona
  2. Minimum Loan Amount of $25,000
  3. Required to open up primary business operating account with Metro Phoenix Bank
    1. Fill out the attached “New Account Request” form (both pages)
    2. Copies of the business documentation, such as Articles of Incorporation/Organization, LLC Operating Agreement, IRS letter issuing the EIN or W9.
    3. Copies of driver’s license of all signers on the account
    4. Copies of driver’s license of all beneficiary owners over 25% if they are not signers on the account

Metro Phoenix Bank has established an online portal to complete the PPP loan application and upload the required documentation. Metro Phoenix Bank will only be accepting applications through this online platform so please do not email or drop off applications/documentation, as these will not be processed.

You will be required to set up an account and the platform will walk you through a step by step process to complete the PPP loan application and upload the required documentation.
